Appointments

To make an appointment, please call the appointment desk at 610-935-1134, extension 127 or 121. You must have an appointment to be guaranteed a visit with one of our providers. The appointment desk is staffed 9:00am-3:00pm, Monday through Friday, but if your call goes to voicemail, please leave a message including your name, phone number and date of birth and we will call you back as soon as possible. If you must cancel an appointment, please do so at least 24 hours ahead of your scheduled appointment time.

New Patient eligibility requirements:

Our services are available at no cost to uninsured adults and children.

 

To qualify you must meet the following:

• Unable to obtain health insurance, including Medicaid, Medicare, CHIP, and VA benefits

• Have a household income below 300% of the Federal Poverty Level

• Live in Chester, Montgomery, or Delaware County

​Our patients must have:

1. Photo identification and proof of current address
2. Contact information for us to reach you (telephone number and/or email)

If English is not your first language, Phoenixville Free Clinic has a Spanish and Portuguese translator available daily, and a language line that has access to over 240 languages.
